Galician primary care reform and medical staffing protests
Overview
Health authorities in Galicia (Sergas) and medical unions have been negotiating a new remuneration model for primary care professionals. The proposal seeks to shift salary structures from a system based on the number of assigned health cards to one based on patient complexity and age. Under this model, patients over 80 would count three times as much as standard patients to reflect higher care intensity. While the Sociedad Española de Médicos Generales y de Familia (SEMG) viewed the move as an advance, they cautioned that financial incentives might not resolve staffing shortages in rural areas without improvements to working conditions.
By mid-October, medical associations expressed support for the Xunta de Galicia’s primary care reform plan for 2026-2030. Backed by a 320 million euro investment, the plan aims to create 644 professional posts by 2030, including 165 new medical positions. The reform is intended to reduce administrative burdens and was described by Pilar Garzón, president of the Consello Galego de Colexios Médicos, as a result of professional consensus.
On October 20, Health Minister Antonio Gómez Caamaño presented a structural reform for medical schedules. Under this plan, family doctors will follow a strictly timed 420-minute workday, including 300 minutes for direct patient care and 100 minutes for training and research. The reform is supported by an expected 6.1% increase in primary care investment by 2027, totaling 1,854 million euros.
Despite these reforms, healthcare advocacy groups have organized protests to denounce critical service shortages. In Xove, the Plataforma Sanitaria da Mariña protested the month-long absence of a pediatrician, while in A Guarda, the Plataforma en Defensa de la Sanidade Pública da Guarda organized demonstrations regarding 2,400 residents lacking a reference doctor.
