German long-term care insurance and assessment processes

Overview

In Germany, long-term care insurance (Pflegekasse) regulations dictate how care levels (Pflegegrad) are assigned and adjusted. Insurers may only lower a care level if there is a significant change in an insured person’s need for assistance, requiring concrete evidence of improved independence in daily tasks and a new assessment by the Medical Service (MD). To secure higher care levels, applicants are advised to document all required assistance thoroughly before evaluations.

For conditions such as dementia, which causes progressive decline in mental abilities, care levels are determined by the Medical Service based on functional needs rather than specific diagnoses. Individuals with severe forms of dementia, such as Alzheimer’s, frequently receive care level 3 upon initial classification. While families often provide early-stage support, the increasing physical and mental demands of caregiving may eventually require a transition to full-time inpatient nursing facilities.
