Healthcare billing fraud in Germany

Overview

The KKH Kaufmännische Krankenkasse reported over eight million euros in losses due to billing fraud during 2024 and 2025. The fraud involved forged prescriptions, unrendered nursing services, and the use of unqualified personnel. Investigators identified “mafiösen Strukturen” (mafia-like structures) operating across federal states and internationally through shell companies.

Following these reports, the GKV-Spitzenverband, the central association of statutory health insurance funds in Germany, commissioned a study to investigate the full extent of undetected misconduct. Based on international research suggesting fraud accounts for five to ten percent of healthcare expenditures, the association estimates potential losses could exceed 20 billion euros, given that annual statutory health insurance expenditures in Germany surpass 350 billion euros.
