HIV medical advancements and global health challenges

Medical research has shown significant progress in managing HIV, including reports of remission in 13 patients following allogeneic hematopoietic stem cell transplants. These procedures utilized donors with the CCR5-delta32 genetic mutation to provide virus resistance, with some patients maintaining remission for over six years after stopping antiretroviral therapy.

Further developments in pharmacology have introduced long-acting injectable formulations that may replace daily oral medications, improving patient quality of life and privacy. While research continues toward a definitive cure through genetic manipulation, health organizations emphasize the need for equitable access to these innovations.

Despite these clinical advances, global health concerns persist. UNAIDS has warned of a potential epidemic resurgence due to declining international funding and cuts to prevention services. In Argentina, budget reductions have specifically impacted programs for HIV, tuberculosis, and viral hepatitis, affecting patient stability and access to essential treatments.

Recent observations indicate that while antiretroviral therapies (ART) have transformed HIV into a manageable chronic disease by allowing patients to achieve undetectable viral loads, this progress has influenced behavioral risks. There has been a noted decline in preventive measures, such as condom use, among both young people and adults over 50. Additionally, because treatments do not eliminate the latent viral reservoir, patients face risks of HIV-associated neurocognitive disorder (HAND). This condition affects the central nervous system, with symptoms ranging from asymptomatic impairment to dementia, impacting quality of life and employment.
