Indonesia BPJS health system reforms
Overview
The Indonesian Ministry of Health has initiated reforms to the BPJS Kesehatan system, focusing on both referral structures and preventative care. A pilot program was launched in Bandung, Makassar, Tulungagung, and Muara Enim to test a competency-based referral system. This model replaces the traditional hospital-class tiers with service capability classifications—Basic, Intermediate, Main, and Comprehensive—to allow patients to be directed to facilities capable of treating specific conditions more efficiently.
In tandem with structural changes, the government is emphasizing active case management through the Free Health Check (CKG) program. Following the detection of 8.3 million cases of diabetes and hypertension, approximately 907,000 individuals have entered treatment. Health Minister Budi Gunadi Sadikin confirmed that there are currently no plans to increase BPJS contribution rates, noting that the government remains committed to covering potential deficits. The ministry’s broader objective is to increase national life expectancy from 72 to 76 years.
