Concerns regarding the stability and coverage of the 118 emergency medical service continue to escalate in several Italian regions. In Puglia, the regional government has approved 2026 staffing plans to hire 1,012 new personnel, including 480 doctors and 42 nurses, in an effort to reduce waiting lists. Despite this, the healthcare system faces intense criticism. AIOP Puglia warned that increasing rehabilitation tariffs without raising spending caps could reduce services, while medical unions like FIMMG Puglia have denounced regional resolutions, claiming changes to physician autonomy and allowances could trigger a “mass exodus of doctors.” In the Capitanata area, reorganization efforts to suppress secondary ambulance stations in locations such as Vieste, Manfredonia, and Cerignola have drawn condemnation. Regional councilor Napoleone Cera alleged that temporary staff were hired for the election period only to have contracts terminated afterward. In Foggia, unions FP CGIL and FP CISL warned that the expiration of contracts for 108 auxiliary workers at Sanitaservice ASL Foggia could lead to a collapse of essential support services. In response to these concerns, the Puglia Region has authorized the ASL of Foggia to begin public selection procedures for 28 permanent full-time positions at Sanitaservice. This hiring plan, which includes 16 auxiliary staff and cleaners, nine emergency responders, and three ambulance drivers, aims to “ensure the continuity of emergency and urgent services and prevent interruptions to public service,” provided the hires do not increase the company’s budget costs. Labor disputes have also emerged regarding meal voucher disparities in the ASL of Taranto. Furthermore, the implementation of the National Recovery and Resilience Plan (PNRR) in Puglia faces delays; while community houses are being built, many remain non-functional due to a lack of medical personnel. These regional issues reflect a broader national crisis in the Italian health service, characterized by structural decay, burnout, and a migration of professionals toward the private sector or Northern Italy.
