In Italy, discussions regarding the accessibility of elective egg freezing, or “social freezing,” have intensified due to high costs and declining birth rates. While medical egg freezing is covered by the national health system, elective procedures remain a significant financial burden, typically costing between 3,000 and 6,000 euros per procedure plus storage fees. Legislative efforts to mitigate these costs have emerged at both the national and regional levels. A national proposal introduced by deputy Emiliano Fossi suggests a 30 million euro annual fund for 2027–2029 to provide subsidies of up to 3,000 euros for women under 40. Regionally, Puglia has offered contributions for specific age groups and income levels, while Tuscany is reviewing a proposal to provide free egg freezing for personal choice. Other regions, such as Sicily and Lombardy, have also implemented or explored various forms of financial aid and vouchers to reduce regional disparities in reproductive healthcare access. However, the expansion of these subsidies faces political opposition in some regions. In Emilia-Romagna, Nicola Marcello, Vice President of the Health Policy Commission, has opposed generalized financial contributions, characterizing them as “electoral handouts” rather than sound health policy. Citing data that only 27% of women who pay for gamete freezing use them later, Marcello argues that broad funding could waste public resources and encourage the postponement of motherhood to ages with higher medical risks. He instead proposes targeting free egg freezing specifically to women with a documented risk of premature ovarian reserve reduction through hormonal and ultrasound screenings. Recent developments show Puglia moving forward with its regional support. A law approved in late 2024 will provide financial assistance for non-medical egg freezing starting in 2025. The regional government will allocate 300,000 euros annually from 2025 to 2027 to offset cryopreservation costs.
