KRAS mutation cancer research and therapy development
Overview
Research into KRAS-targeted cancer therapies has focused on identifying the biological mechanisms behind drug resistance and the varying efficacy of treatments across different mutation subtypes.
Initial studies using mouse models identified two primary ways tumors evade drugs: by increasing the number of KRAS gene copies to intensify growth signals, or by undergoing a cellular identity shift from adenocarcinomas to squamous cell carcinomas. This shift reduces dependency on KRAS, allowing growth through alternative signaling pathways.
Subsequent clinical developments have highlighted challenges in treating specific subtypes. While combining KRAS G12C inhibitors with anti-EGFR antibodies has improved progression-free survival in colorectal cancer, data from pan-RAS inhibitor trials show significant differences in response rates. For example, the drug daraxonrasib showed a 50% response rate for the G12V subtype but only 24% for the G12D subtype. This disparity has driven increased industry focus and investment into highly selective KRAS G12D inhibitors.
