Rising stroke incidence among young US adults

Overview

Research published in the journal Neurology by University of Cincinnati scientists indicates a significant increase in stroke incidence among adults aged 20 to 54. Analyzing data from the Cincinnati and Northern Kentucky regions between 1993 and 2020, the study found that stroke rates rose from 33.9 to 62.2 cases per 100,000 person-years, an increase of approximately 83 percent.

The surge is primarily driven by ischemic strokes, which occur when blood vessels supplying the brain become obstructed. While ischemic stroke rates rose significantly, hemorrhagic stroke rates did not show a similar increase.

Key risk factors identified in younger patients include hypertension, diabetes, atrial fibrillation, and psychoactive substance use. Notably, the proportion of patients reporting substance use—largely involving marijuana—rose from 4.6 percent to 40.2 percent. Although 30-day mortality rates decreased from 11.7 percent to 9.4 percent, researchers noted that more young people are surviving but living with long-term consequences such as impaired movement, speech, or memory.
