Silver Diamine Fluoride trial in US children

Overview

A Phase III clinical trial involving 830 children under six in Michigan, New York, and Iowa has demonstrated that silver diamine fluoride (SDF) can effectively halt cavity progression. Reported in JAMA Pediatrics, the study led by Professor Margherita Fontana found that a single application stopped decay in 54% of treated teeth, compared to 22.5% in the placebo group. The treatment was notably effective on front teeth, where 42% of lesions were arrested, versus 26% on back teeth.

SDF offers a low-cost, non-invasive alternative to traditional drilling, anesthesia, or sedation, making it potentially vital for children with dental anxiety, special health needs, or limited access to care. While the liquid is highly effective, it causes permanent black staining of the treated tooth tissue. Researchers noted this cosmetic drawback may be less concerning for primary teeth, as they are eventually replaced by permanent teeth.

While SDF is on the WHO essential medicines list, it is currently only cleared in the US for treating tooth sensitivity. This large-scale data could support FDA approval for broader pediatric use. The trial’s findings coincide with international concerns regarding pediatric dental health, such as rising tooth-decay rates and hospital admissions in the United Kingdom.
