In South Korea, comprehensive training hospitals have established a consultative body to advocate for improved institutional status and increased financial support. The group seeks to address the economic challenges of transitioning from a resident-dependent model to a specialist-centered medical system, arguing that government support is disproportionately concentrated on tertiary general hospitals. They have proposed a tiered support system for secondary general hospitals to ensure they can effectively serve as regional centers for high-difficulty medical services.

Building on these efforts, surgical associations and secondary hospitals have formed a Hospital Committee to address financial instability within regional surgical services. This group is specifically challenging the current Diagnosis Related Group (DRG) system, arguing that uniform compensation fails to account for clinical variations and surgical difficulty. They are calling for reimbursement models that reflect actual medical costs, emergency status, and the fixed expenses required to maintain 24-hour surgical teams.
