Internal records and call recordings show that of the ten air traffic controllers on duty during that incident, five were flagged as being at ‘high fatigue risk’. Of the three controllers directly involved in the near-miss, two were rated at high fatigue and one at medium fatigue. During an internal call, a shift manager described the staffing situation as ‘a bit insane.’

Other incidents under investigation include an August 9 emergency braking event involving a Jetstar aircraft and a Qatar Airways Boeing 777, which resulted in a crew member sustaining injuries, and an August 16 event where a Virgin Australia Boeing 737 braked to avoid a Qantas 737. An August 25 incident involved a Jetstar Airbus A320 crew halting a pushback due to incomplete taxi information regarding a nearby Bombardier Dash 8.

Staffing shortages have since caused operational disruptions. On a recent Friday morning, Airservices Australia implemented Air Traffic Flow Management (ATFM) measures due to roster constraints, resulting in six flight cancellations and delays for several Qantas flights. Airservices Australia stated it is working to recruit more controllers, and executives are scheduled to appear before a Senate committee to discuss the state of the aviation sector.

The ATSB continues to review flight recordings and interview personnel to identify common contributing factors. Airservices Australia has declined to comment due to the ongoing investigation.
