Dominican Republic launches High-Cost Medication Fund

Overview

The High-Cost Medication Fund (FOMAC) has officially launched in the Dominican Republic, providing 100% coverage for essential medications without copayments. Approved by the National Council of Social Security (CNSS) and presented by the Ministry of Public Health and SISALRIL, the fund targets approximately 6,400 affiliates in its initial phase. The program covers 13 key drugs for 16 catastrophic diseases, including various cancers, rheumatoid arthritis, psoriasis, and spinal muscular atrophy. To ensure financial sustainability, the system utilizes centralized volume purchasing to generate savings, with funding drawn directly from the Family Health Insurance (SFS) system without requiring additional contributions from workers or employers. The implementation includes a transition and operational capitalization period of up to twelve months.

Following the launch, the Dominican Society of Rheumatology (SDR) has called for the expansion of the fund to include a broader range of rheumatic and immunomediated conditions. While acknowledging the inclusion of six relevant medications for rheumatic diseases in the current phase, the SDR emphasized that true benefit depends on access conditions, eligibility, and continuous supply. The society proposed adding coverage for conditions such as systemic lupus erythematosus, ankylosing sponditis, scleroderma, Sjögren's disease, high-risk osteoporosis, multiple sclerosis, and systemic vasculitis in future stages.
