Healthcare resource and labor issues in Mexico and El_Salv

Overview

Medical professionals in Mexico and El Salvador have reported significant challenges regarding hospital resources and labor conditions. In El Salvador, healthcare workers represented by the ‘Dr. Salvador Allende’ Health Movement of ALAMES have denounced poor working conditions, including arbitrary schedule changes and restrictive policies regarding personal items.

In Mexico, the situation has escalated into a broader crisis characterized by structural failures and non-compliance with constitutional mandates for the right to health. Since October 2026, medical, nursing, and unionized personnel, particularly from the IMSS, have engaged in protests and labor strikes. These actions respond to chronic shortages of essential medicines, medical supplies, serums, and oxygen, as well as insufficient wages and a lack of specialized infrastructure.

To address supply chain delays, President Claudia Sheinbaum has announced a plan to modernize and digitalize Mexico’s public health system, beginning with IMSS-Bienestar. The initiative seeks to eliminate manual paperwork and physical memos. To ensure traceability from suppliers to patients, the government intends to recruit specialists in data science and logistics to develop automated warehouses and digital tracking systems. Parallel to this, the administration is expanding physical infrastructure, including the rehabilitation of the General Hospital in Ciudad del Carmen, Campeche, and the planned installation of 905 new health clinics across 125 municipalities in the State of Mexico by 2027.

Compounding these issues is a significant deficit in medical personnel relative to OECD standards. Mexico maintains a density of roughly 2.4 to 2.7 doctors per 1,000 inhabitants, approximately 31% below the OECD average. This shortage is marked by severe geographic inequality, with professionals concentrated in metropolitan areas while southern states like Chiapas, Oaxaca, and Guerrero face precarious coverage. Additionally, a lack of specialists—with only 33% of doctors holding specialized training—limits access to high-level care in rural and suburban communities.
