A legal and criminal crisis has emerged regarding the illicit trade of tirzepatide, the active ingredient in weight-loss medications, across the border regions of Paraguay and Brazil.

In Paraguay, a criminal complaint was filed in Ciudad del Este by a U.S. entrepreneur against the pharmaceutical firm Indufar C.I.S.A. The lawsuit alleges fraud, breach of trust, and criminal conspiracy related to the development of a generic version of the drug, claiming losses exceeding $4 million. Concurrently, Ciudad del Este has emerged as a major hub for smuggling tirzepatide into Brazil, with black-market revenues estimated at $485 million.

In Brazil, the investigation known as ‘Operação Off Label’ has progressed through multiple phases. In Paraná, the Public Ministry (MPPR) filed charges against eight individuals, including a retired civil police officer and a suspended active-duty officer. The investigation, triggered by a January 2026 seizure in Londrina, alleges the officers coordinated the theft of medication shipments—using a firearm to threaten a manager—to be resold via WhatsApp groups.

Smuggling activity continues to escalate. In August 2026, authorities in Irati seized 1,200 ampoules of the foreign-sourced drug, valued at R$ 135,000, hidden inside a vehicle’s spare tire. In Rolândia, police intercepted local council members while they were transporting tirzepatide and other contraband from Paraguay.

In September 2026, the Federal Highway Police (PRF) conducted further major seizures. In Marmeleiro, Paraná, authorities discovered approximately 9,600 vials hidden in a false bottom of a trailer. In Itatiaia, Rio de Janeiro, officers seized roughly 655 vials on the Presidente Dutra highway; the driver fled the scene. Authorities noted that the seized medication lacked the continuous refrigeration required for safe transport. The Federal Revenue Service reported that seizures at the Brazil-Paraguay border reached record levels, with over 76,000 confiscations recorded in the first half of 2026.
