Inclisiran efficacy and evolving LDL cholesterol treatments
Overview
Research into the efficacy of inclisiran, a drug utilizing small interfering RNA (siRNA) technology to inhibit PCSK9 protein production, has demonstrated significant potential in reducing LDL cholesterol.
An initial meta-analysis of nine randomized clinical trials involving over 6,300 participants found that inclisiran achieved an average LDL cholesterol reduction of approximately 49% compared to a placebo. The drug’s infrequent dosing schedule—requiring an injection only once every six months after initial doses—could improve patient compliance compared to daily statin regimens.
Results from the Victorion-Challenge study, presented at the European Society of Cardiology (ESC) Congress in Munich, further established inclisiran’s efficacy in patients with high or very high cardiovascular risk from atherosclerotic cardiovascular disease (Ascvd). In this phase IV, randomized, open-label study, the inclisiran group saw a mean LDL-C reduction of 56.31% at day 150, compared to a 15.35% reduction in the bempedoic acid group. Notably, 78.2% of patients treated with inclisiran reached their target LDL-C levels, while only 20.3% of those on bempedoic acid achieved the same goal. Despite these differences in efficacy, the frequency of treatment-related adverse events was similar between the two groups, reported at 63.1% for inclisiran and 59.8% for bempedoic acid.
The landscape for cholesterol management is expanding with the FDA approval of Lipfendra (enlicitide), the first oral PCSK9 inhibitor. Expected to be available by 2026, this medication provides an oral alternative to injectable treatments for high-risk adults.
