Increase in healthcare proxies in Belgium

Overview

During the first half of 2026, Belgium experienced a 38.6 percent increase in the number of healthcare proxies and care mandates established compared to the same period in 2025. Data from the Federation of Notaries (Fednot) indicates that a total of 92,026 mandates were signed.

The majority of these arrangements occurred in Flanders, which accounted for 91 percent of the total with 83,787 mandates. Wallonia and the Brussels-Capital Region also saw growth, recording 5,717 and 2,522 mandates, respectively.

These legal instruments enable individuals to designate persons to manage financial affairs—such as pensions, bills, and bank accounts—and long-term care preferences in the event of mental incapacity due to illness or accident. Upcoming legislation scheduled for 2027 is expected to limit the role of estate managers to close family members or registered professionals.
