Mexico-Paraguay fiscal and healthcare reform initiatives

In July 2026, analysts highlighted the deep fiscal strains facing both Mexico and Paraguay. Mexico was urged to launch a comprehensive fiscal overhaul to halt the erosion of public budgets, raise public investment to 2.5% of GDP, and address costly legacy projects such as the AIFA airport, Tren Maya, the Interoceánico rail line and the Dos Bocas refinery, while state utilities Pemex and CFE continued to drain finances. Paraguay’s public pension fund, the Caja Fiscal, reported a large deficit, with payouts far exceeding contributions, underscoring the limited impact of recent pension-reform legislation.

By early August 2026, legislators in both countries moved toward concrete budget reforms aimed at improving public-health financing. In Mexico, Ivonne Ortega Pacheco, coordinator of the Movimiento Ciudadano parliamentary group, introduced a bill to amend the Financial Discipline Law to explicitly cover health emergencies under Article 7. This would allow state and municipal governments to make extraordinary health-related expenditures without bureaucratic delays. In Paraguay, a legislative project sought to reallocate at least 500 billion guaraníes from existing budget lines to the Ministry of Health for medicines procurement, emphasizing that the move would not increase the overall budget or fiscal deficit.

By late August 2026, new proposals emerged to further shift healthcare funding. In Paraguay, Senator Eduardo Nakayama revived a proposal to eliminate private medical insurance for public officials, aiming to redirect an estimated USD 100 million to USD 120 million annually toward the public health system. In Mexico, Deputy Noel Fernández Maturino called for the federal government to ensure sufficient resources in the 2027 Federal Expenditure Budget to sustain healthcare services, following a 270 million peso investment by the Durango state government to improve medical supplies and prosthetics.
