About 15‑20 % of allocated slots are relinquished each year, allowing waiting patients to be offered a turn within days rather than the usual 12‑18‑month wait. To participate, patients must hold a confirmed NFZ referral, submit a statement of readiness to the regional office and be prepared to travel within roughly 14 days of an offer; acceptance must be confirmed within 24‑48 hours. The fund warns that paid services promising faster access are scams.

The scheme covers popular spa towns such as Zakopane, Krynica‑Zdrój, Ustroń, Kołobrzeg and others where queues are longest. NFZ officials note that the returns system offers a practical alternative for the roughly 800 KRUS places granted annually to retired farmers, whose backlog exceeds 12,000 applicants and can entail waits of up to 20 years.
