Overview

Rete Ferroviaria Italiana (Rfi) has faced legal and regulatory scrutiny following significant safety and operational failures.

In relation to the Brandizzo train tragedy, which resulted in the deaths of five workers on the Torino-Chivasso line, the Ivrea Prosecutor’s Office has placed two Rfi executives under investigation. An inspection revealed that the company’s Risk Assessment Document had not been updated or implemented according to required timelines. Evidence suggests workers were sent onto tracks without certainty that rail traffic had been suspended; one subcontractor supervisor was recorded telling his team, “Guys, if I say train, jump aside.”

Separately, the Italian Transport Authority imposed a 1.75 million euro fine on Rfi due to a major rail traffic shutdown at Rome Termini. The authority cited negligence in infrastructure maintenance and management regarding an incident caused by a nail that paralyzed services. Rfi acknowledged the decision but argued the incident was caused by an external contractor performing night works.
