Rural pharmacy healthcare services in Spain

Community pharmacies in rural Spain are expanding their roles to provide essential healthcare services to aging populations in areas facing depopulation.

In Granada, the ‘Mi Reto’ program has been renewed to offer free pharmaceutical assistance to seniors, including monitoring clinical parameters and reviewing home medicine cabinets. In Albacete, the MediCAMProx project has enabled 742 patients in depopulated areas to receive hospital-dispensed medications locally, supported by increased digitalization through electronic prescriptions. In León, pharmacies act as vital hubs providing continuous professional access where medical center hours are limited. The regional pharmacy council has launched the campaign ‘Rural pharmacy. Much more than rural cohesion’ to emphasize their role as primary healthcare resources.

In Galicia, nearly 80% of the 1,384 pharmacies are located outside provincial capitals. This network has implemented electronic prescriptions for MUFACE mutualists and launched the ‘sentinel pharmacy’ project, utilizing 70 locations to monitor epidemiological trends such as influenza. Similarly, in Extremadura, where 84.7% of pharmacies are outside provincial capitals, these establishments serve as the primary entry point to the health system in many small municipalities.

Recent seasonal trends in Extremadura have introduced new logistical pressures. During summer months, rural towns experience significant population surges as residents return from abroad and visitors arrive for local festivities, sometimes doubling or tripling the local population. In municipalities such as Pasarón de la Vera and Villamiel, pharmacists report that the volume of prescriptions can triple during August, often preventing them from taking vacations due to the saturation of local health centers.

To address economic sustainability and ensure the supply of essential medicines, Ourense has declared ‘special pharmaceutical zones’ to provide economic support. Despite these advancements, the sector continues to face challenges regarding economic viability and the need for professional recognition under upcoming medication laws.
