Overview
The STAREE trial, a large-scale clinical study involving nearly 10,000 participants from Australian general practices, has demonstrated that daily atorvastatin significantly reduces the risk of major cardiovascular events in healthy adults aged 70 and older.
Findings published in the New England Journal of Medicine indicate a 30 per cent reduction in the risk of cardiovascular death, nonfatal myocardial infarction, stroke, and coronary revascularization among participants with no prior history of cardiovascular disease, diabetes, or dementia. In absolute terms, this represented a reduction of approximately 2 to 3 events per 100 people treated over a median follow-up of 5.9 years. While the medication successfully lowered cardiovascular risks, researchers noted that it did not significantly improve mortality, dementia, or disability-free survival.
Recent research has also expanded the understanding of cholesterol management. While statins inhibit cholesterol synthesis, studies into PCSK9 inhibitors, such as evolocumab and alirocumab, highlight their role in treating severe hypercholesterolemia. These inhibitors work by blocking a liver-produced protein, increasing the liver’s ability to remove LDL cholesterol from the bloodstream, offering an alternative for patients unable to reach therapeutic targets through statins alone.
Additionally, scientists at the University of British Columbia and the University of Wisconsin–Madison have identified a potential biological cause for statin-induced muscle pain. Using cryo-electron microscopy, they discovered that statins may attach to the RyR1 protein, disrupting calcium levels necessary for muscle function. This aligns with observations from the STAREE trial, where musculoskeletal issues were slightly more frequent in the atorvastatin group than in the placebo group.
