Dominican Republic health insurance regulation and expansion
Overview
The Superintendency of Health and Labor Risks (SISALRIL) in the Dominican Republic has taken steps to regulate healthcare services and expand coverage.
Initially, SISALRIL issued instructions to Health Risk Administrators (ARS) and healthcare providers to prevent contractual or financial disputes from disrupting services for Family Health Insurance (SFS) members. The regulator mandated that disputes regarding rates or payments must be resolved through institutional channels and prohibited the withholding of documents or unjustified service suspensions. Additionally, providers were required to issue receipts for Basic Health Plan services and offer electronic payment options to improve transparency.
Through Administrative Resolution No. 00269, SISALRIL expanded health benefits for over 122,000 state retirees and pensioners. This expansion includes new coverage for treatments such as gigantomastia, gynecomastia, tuberculosis medications, and antiretroviral drugs for HIV. The measure also increased payment rates for outpatient services in dentistry, psychology, and psychiatry. Superintendent Miguel Ceara Hatton stated the expansion is intended to “close the gap between the Special Plans and the Contributory Regime.” To ensure financing, the Social Security Treasury (TSS) was instructed to manage the necessary fund dispersions.
Furthering its regulatory scope, SISALRIL introduced a new Schedule of Occupational Diseases via Resolution 00270-26 to expand Labor Risks Insurance (SRL) coverage. This update modernizes worker protections by linking diagnoses to specific job functions. For the first time, coverage includes mental and psychosocial conditions such as burnout syndrome, depressive episodes, and non-organic sleep disorders. The expansion also incorporates various occupational cancers—including lung, stomach, and prostate—and organizes pathologies into seven groups based on causal agents and work-related exposure. The Dominican Institute for the Prevention and Protection of Occupational Risks (Idoppril) will manage the medical costs and disability registrations resulting from these recognized conditions.
