Eli Lilly retatrutide development and black market issues
Overview
In early August 2026, Eli Lilly announced that its experimental obesity treatment retatrutide achieved substantial weight-loss outcomes in two phase-3 trials. Participants lost up to 22.6% of body weight over 80 weeks in a study involving individuals with severe obesity and cardiovascular disease, while a separate study involving individuals with obesity but without diabetes saw average weight loss reach 28.3% at the highest dose. The drug’s triple-agonist approach—targeting GLP-1, GIP, and glucagon—may influence how the body utilizes energy rather than just suppressing appetite. Common side effects include nausea and diarrhea, and a small subgroup showed a higher incidence of serious cardiovascular events.
Eli Lilly plans to seek FDA approval in the United States during the first quarter of 2027. To provide access, Lilly launched a compassionate-use program in the U.S. for adults with refractory obesity and at least two serious weight-related complications who are ineligible for ongoing trials.
As the drug remains in the clinical trial phase without regulatory approval, counterfeit versions are flooding black markets. In the UK, illicit vials are reportedly being distributed through gyms, beauty salons, hairdressers, social media, and street vendors. Health professionals have expressed concern regarding individuals injecting these unregulated, illegal substances. Consequently, Eli Lilly has initiated legal action against various U.S. entities, including compounding pharmacies and medical spas, for the illegal sale of black-market retatrutide. The company has referred over 200 entities to the FDA and the Department of Justice, flagging more than 14,000 illegal advertisements and product listings globally.
