Healthcare administration and service reforms in Puglia
Overview
Healthcare administration in Puglia is undergoing significant regulatory and structural changes. To address service delays, the regional government has approved the second phase of an experimental structural plan to reduce healthcare waiting lists, supported by 20 million euros in funding. This follows a first phase that concluded on June 30, 2026, which exceeded its target by recovering 157,176 medical services and contacting 231,435 citizens.
The current phase aims to consolidate these results by focusing on services that fail to meet established timeframes. Strict maximum wait times have been confirmed: 72 hours for class U (urgent), 10 days for class B (short-term), 30 days for class D visits, 60 days for class D diagnostics, and 120 days for class P (programmable). The plan also includes facilitated pathways for oncology patients and those with rare diseases.
New prescription rules require doctors to specify the diagnostic reason and distinguish between first-time and follow-up access. Urgent prescriptions now have specific expiration dates: 5 days for code U and 15 days for code B.
The initiative has faced criticism from the Fratelli d'Italia (FdI) group. Representatives, including Paolo Pagliaro, argue that the plan utilizes “accounting tricks” to reduce lists by allowing urgent prescriptions to expire rather than addressing underlying service shortages. They further claim the regulations increase bureaucracy for family doctors without effectively meeting patient needs.
To strengthen territorial assistance, the Ministry of Health has authorized the activation of the 116117 Harmonized European Number in Puglia, providing 24/7 non-urgent medical assistance to complement existing emergency services.
