The first phase will examine the treatment of the Nowak family by various agencies and investigate whether public services—including the police, healthcare, local authorities, universities, and religious organizations—missed warning signs regarding Vickrum Digwa in the five years preceding the attack. The second phase will consider the actions of the police and the Independent Office for Police Conduct (IOPC) once their investigation into the scene response is complete. An initial report is expected within five months.
