German health reforms: caps, GP shortage, psychotherapy cuts

Entities: Kassenärztliche Vereinigung Rheinland-Pfalz · Andreas Bartels · German Federal Ministry of Health

Overview

Protests against the June 2026 GKV Savings Act have continued as the black‑red coalition enacted an €18.8 billion health‑spending savings package. The law caps expenditures for doctors’ practices, clinics, pharmacies and the pharmaceutical sector, raises co‑payments for medicines, limits free spouse coverage and keeps the statutory insurance contribution rate at 17.5 % of gross wages. Health‑minister Nina Warken says the measures will close the projected 2027 financing gap; a €25.3 billion mini‑buffer for 2028 and further relief packages for 2029‑2030 are also planned. The shortage of physicians remains acute. A Bosch Health Campus study released in July 2026 confirms earlier forecasts that roughly 12,000 general‑practice positions will be vacant by 2040 – about one‑fifth of all GP seats. More than 4,400 GP posts are already unfilled, the average age of practising GPs has risen from 52.6 to 55 years, and the share over 65 has climbed from 6 % to 16.5 %. Rural districts such as Hochsauerlandkreis, Paderborn and Minden‑Lübbecke face projected drops of around 47‑48 % in GP density, with about 20 % of counties expected to fall below a 75 % coverage threshold. Total GP numbers have risen only four percent since 2019, lagging behind growth in other specialties. Psychotherapy financing is under scrutiny. Green health‑policy spokesperson Paula Piechotta warned that the planned €3.8 billion annual budgeting of psychotherapeutic services could lengthen already long waiting times and argued that cuts should target drug costs instead. She called for stronger preventive measures as mental‑health cases rise. The Kassenärztliche Vereinigung in Rhineland‑Palatinate warned that the new funding law will cut financing for doctors’ practices, clinics, pharmacies and the pharmaceutical sector, making it harder for new physicians to take over practices, especially in rural areas.
