US Afrikaner refugee policy and PEPFAR funding cliff

Overview

In May 2026 the Trump administration set aside refugee slots for white Afrikaners, later expanding to 17,500 and linking the move to alleged racially‑motivated violence. Simultaneously it announced a phased withdrawal of roughly $400 million‑per‑year in PEPFAR funding from South Africa, citing Pretoria’s race‑based policies. South Africa’s health ministry disputed formal notice but noted it already finances most antiretroviral drugs. UNAIDS warned that the cut could jeopardise treatment for the world’s largest HIV‑positive population. The State Department formally notified South Africa on 22 June and detailed “welcome bags” for incoming refugees, including an Android tablet, U.S. flag, Constitution, Declaration of Independence, PragerU material and a 1776 Commission report. In July 2026 the administration announced the expiration of 120 CDC‑administered PEPFAR awards in September, creating a funding cliff for services supporting over 8.7 million HIV patients worldwide. A Boeing 767‑300 flew 59 South African Afrikaner refugees to Washington, DC, at a cost exceeding $400 000, marking the first large‑scale implementation of the scheme, which had admitted 7,727 South African nationals in FY 2025‑26.

A July 2026 report by HIV charity amfAR, based on a survey of 166 organisations in 46 countries receiving PEPFAR aid, found the cuts led to the closure of more than 1,700 HIV clinics and the loss of over 16,000 staff. About three‑quarters of respondents stopped services for high‑risk groups such as sex workers, MSM, transgender people and people who inject drugs. Paediatric treatment fell by roughly 77,000 children, with South Africa accounting for the largest drop of around 30,000. UNAIDS warned that permanent reductions could generate an extra 6.6 million infections and 4.2 million AIDS deaths by 2029. France, Germany and the United Kingdom also scaled back aid, compounding pressure on health programmes across Africa and other regions.
