By mid-2026, a contractual deadlock between regional health authorities and private centres—stemming from a change in regional government and budgeting delays—led to warnings of potential service suspensions. Facilities have been operating on informal extensions of undersized 2025 volumes, forcing them to self-fund salaries and maintenance. A July 2026 meeting at the Hotel Ramada in Naples failed to resolve these programming issues. Regional councilor Vincenzo Santangelo warned that the lack of secured resources could interrupt radiotherapy for up to 15,000 cancer patients, calling for an emergency safeguard measure and a permanent technical-political board.

Simultaneously, the National Union of Cooperative Health Enterprises (UNCI Campania) appealed to regional leadership to unblock ASL payments, noting that outstanding 2025 balances and salary delays jeopardize care for elderly and disabled patients.

By late August 2026, the crisis expanded. The association Aiop signaled that private accredited facilities may suspend rehabilitation, long-term care, elective surgeries, and certain outpatient services starting September 15 if financial solutions are not provided. While emergency services would remain, the move would shift pressure to public hospitals. In response, labor unions Uil Fp and Fp Cgil have declared a state of agitation, demanding institutional meetings to protect workers' rights and address the budget disputes.
