German GKV reform faces funding gap and primary care shifts
Overview
The implementation of the GKV-Beitragssatzstabilisierungsgesetz has triggered significant financial and structural tensions within the German healthcare system. While the law aims to address a projected deficit of over €18 billion by 2027, insurers like AOK PLUS have warned that it fails to solve structural financing issues. Following the law's passage by the Bundestag and its effectiveness as of July 30, 2026, insured individuals face rising out-of-pocket costs. Medication co-payments are set to increase from a range of 5 to 10 euros to between 7.50 and 15 euros, while daily hospital co-payments will rise from 10 to 15 euros. Additionally, subsidies for dental prosthetics will decrease, and insurance will no longer reimburse cannabis flowers. The reform also introduces stricter budget constraints in outpatient care. In response to these pressures, Klaus Reinhardt, President of the German Medical Association, and the General Practitioners Association are advocating for a mandatory primary care model, potentially to be implemented by 2028. To ensure adherence to new ‘rules of the game,’ Reinhardt suggests that patients who bypass general practitioners to visit specialists directly should face consequences, such as longer waiting times or financial mechanisms, while those who comply could benefit from shorter waits. To optimize physician time, Reinhardt has proposed increasing health literacy through initiatives such as retired physicians visiting schools to teach children how to manage minor ailments. He also emphasized the need to combat bureaucratic workloads and create incentives for older doctors to work in part-time capacities. Public sentiment remains strained; a YouGov survey indicates that 71 percent of citizens find it difficult to secure appointments, while 74 percent desire better digital transparency for available slots. Further complicating the landscape, the insurer IKK Brandenburg and Berlin (IKK BB) has actively challenged the current funding model.
