BPJS Kesehatan administrative and coverage updates

Overview

BPJS Kesehatan has issued clarifications regarding administrative procedures and coverage limitations for the National Health Insurance (JKN) program in Indonesia. Initial updates focused on the regulation of control letters (surat kontrol), which must be issued by authorized medical facilities rather than at a participant's request. Starting 1 September 2026, these letters will no longer be issued on the same day as the initial service. Officials in Jakarta have emphasized that participants must obtain a formal doctor's control letter before leaving a hospital polyclinic to prevent the loss of service quotas due to scheduling errors or specialist availability. Regarding facility management, the agency clarified that while participants can typically only change their primary health facility (FKTP) after three months, exceptions are made for relocations or work assignments. In Jember, officials noted that if a facility's partnership ends due to licensing issues, BPJS Kesehatan will transition participants to other qualified facilities to ensure uninterrupted service. To maintain active membership, non-wage earners must pay monthly premiums by the 10th of each month. As of August 2026, contribution rates remain unchanged under Presidential Regulations No. 59 of 2024 and No. 63 of 2022. To ensure data accuracy, families must deactivate memberships of deceased participants using a death certificate and National Identity Number (NIK) via online services. In September 2026, approximately 178,000 recipients of the Contribution Assistance Recipient (PBI-JK) program in Semarang and Demak lost coverage due to changes in their economic decile status. According to Debbie Nianta Musigiasari, Head of the Semarang Branch of BPJS Kesehatan, the Ministry of Social Affairs issues monthly decrees regarding these deactivations. Only those in deciles 1 through 5 qualify for PBI-JK. Affected citizens may apply for reactivation through local social services within six months of the deactivation decree, pending updated data from the Ministry of Social Affairs.
