Brazil aviation safety under scrutiny

It concluded the aircraft should not have taken off because the de‑icing system was inoperable and the windshield‑wiper faulty, and reiterated a “culture of normalising deviations” within Voepass.
A July 24 update added that repeated ice‑detection alerts were omitted from flight logs, pilots routinely disabled system warnings, and maintenance practices transferred components from faulty aircraft without adequate inspection. The final report emphasized that pilots ignored alerts, maintenance logs showed recurring deficiencies, and oversight by ANAC was lax. It recommends stricter enforcement of de‑icing procedures, mandatory reporting of system failures, and enhanced supervision of airline maintenance practices.
Further detail confirmed the flight – a Cascavel‑to‑São Paulo‑Guarulhos service carrying 58 passengers and 4 crew – crashed near Vinhedo, killing all 62 aboard. Crew recordings showed the captain maintaining altitude despite the copilot’s recommendation to descend, while routine communications distracted the crew as ice accumulated. The agency urged ANAC to overhaul surveillance and risk‑assessment procedures and called on EASA to review ATR’s degraded‑performance guidelines.
