Led by Hagai Levine of the Hebrew University of Jerusalem, the study of over 118,000 men across Israel, the United States, Brazil, Finland, and Denmark noted that this decline, exceeding 1% annually, accelerated after 2000.

Researchers link this trend to obesity, diabetes, metabolic syndrome, and exposure to endocrine-disrupting chemicals. The European Chemicals Agency (ECHA) has identified bisphenols, such as BPA, as reproductive toxins. While the EU is phasing out BPA in food-contact materials, some products may remain on the market until 2028, and substitutes like BPS may also affect male reproductive systems.

Medical experts, including Mikael Lehtihet of the Karolinska Institutet, warn that while more men are seeking evaluations for deficiency, true deficiency is rare among healthy men and often linked to underlying conditions like weight gain or type 2 diabetes. A diagnosis requires repeated low values and specific symptoms, such as a significant loss of sexual desire.

Modern lifestyle factors—including sedentary behavior, chronic stress, poor sleep quality, and diets high in processed foods—are causing younger men to experience low testosterone earlier than in the past. This can impact muscle mass, bone density, and fat distribution. Furthermore, the misuse of anabolic steroids for muscle enhancement poses severe risks, including cardiovascular disease, hypertension, kidney damage, and impaired fertility. To combat declining sperm quality, specialists recommend lifestyle interventions such as a Mediterranean diet and regular exercise.
