Overview

In July 2026 the governing coalition passed a revised statutory health‑insurance (GKV) package that lowered the family‑insurance surcharge, extended parental exemptions, cut federal subsidies, earmarked funds for low‑income care and raised the pharmaceutical rebate. The bill introduced spending caps, higher co‑payments for medicines and hospital stays, and a mandatory 24‑hour cooling‑off for individually billed add‑ons (IGeL). The Bundestag approved the legislation on 10 July, alongside an amendment removing insurers’ duty to notify members of supplemental‑rate increases. State leaders, statutory insurers and doctors’ groups criticised the measures, warning of cost shifts to patients and pressure on care quality. Patient‑protection groups and medical experts called for stricter IGeL regulation, mandatory documentation in electronic health records and transparent pricing. The federal health ministry rejected a blanket ban on IGeL, citing physicians’ therapeutic freedom and placing oversight with regional medical chambers. Green Party politician Armin Grau and consumer advocates continued to press for stronger controls. On 21 July the notification‑duty amendment took effect, prompting the AOK association to warn that members could lose a special termination period and face higher out‑of‑pocket costs. SPD health policymaker Christos Pantazis and financial expert Hermann‑Josef Tenhagen urged a redesign of digital notifications. The Green Party released a comprehensive policy paper proposing broader reforms, including shifting non‑insurance services to tax funding, consolidating hospitals, overhauling emergency care, capping drug prices and strengthening primary physicians. The GKV‑Beitragsstabilisierungsgesetz (BStabG), which came into force on 30 July 2026, aims to cut statutory‑insurance spending by €19 billion by 2030. From 2028 families with childless spouses will face a 2.5‑percentage‑point surcharge, and the free co‑insurance for spouses will be limited. Prescription co‑payments will rise to €7.50‑€15 per item from January 2027.
