Tesla FSD safety scrutiny intensifies
Overview
Since the July 2026 indictment over the June‑19 Model 3 fatal crash in Katy, Texas, Tesla has faced heightened legal exposure in the United States. A NTSB preliminary report confirmed driver Michael Butler overrode Full‑Self‑Driving (Supervised) and floored the accelerator, causing the vehicle to strike a home at over 70 mph and kill Martha Avila. The case fuels ongoing NHTSA investigations and adds to a portfolio of nearly 50 special inquiries into Tesla’s driver‑assistance systems.
In Europe, the Berlin‑area Gigafactory expansion and strong EV adoption have accelerated market growth. The EU Technical Committee’s June 30 meeting will decide continent‑wide type‑approval, with debate over the “Speed Offset” feature. After Dutch approval in early April, five EU members logged more than 50 million km on FSD Supervised within three months, showing lower collision rates than manual driving.
Tesla’s software rollout continues. On 20 July 2026 the company began a wide deployment of FSD v14 Lite to its older HW 3 fleet (≈ four million vehicles), adding start‑from‑park capability, parking‑location icons, speed‑profile settings and refined navigation handling. The same update introduced a major Summer 2026 upgrade to the Grok voice assistant, enabling phone calls, music search, climate control, glove‑box access and expanded navigation learning. The following day Elon Musk announced a future shift to personalized FSD, where the neural network will adapt to individual drivers’ lane‑changing, HOV‑lane and parking habits, addressing user complaints about uniform behavior.
These developments occur alongside the NTSB’s affirmation that driver action, not software, caused the Katy fatality, while regulatory scrutiny and feature expansion proceed in parallel.
